This first-in-human trial tested a closed-loop system that automatically adjusts transcranial focused ultrasound (tFUS) intensity based on real-time EEG signatures of default mode network (DMN) activity in 38 patients with treatment-resistant depression.

The adaptive system outperformed fixed-dose tFUS, achieving 62% response rate vs 31% with conventional protocols.
