Overview

Chronic pain, particularly nociplastic pain, is a common challenge in outpatient practice. Pain reprocessing therapy (PRT) is a therapeutic approach aimed at modifying the brain's interpretation of pain and breaking the fear-pain cycle. Drawing from theories and research on neuroplasticity and central sensitization, PRT proposes reframing symptoms as potentially harmless signals. Recent randomized controlled trials, including one with long-term follow-up, show a significant and sustained reduction in pain. This article summarizes the current research, conceptual foundations, available training programs, therapeutic techniques, and integration of PRT into practice, with an example from the clinical practice at the CHUV. La douleur chronique, notamment nociplastique, constitue un défi fréquent en pratique ambulatoire. La thérapie de reconditionnement de la douleur (pain reprocessing therapy, PRT) est une approche thérapeutique visant à modifier l’interprétation cérébrale de la douleur et à interrompre le cercle vicieux peur-douleur. Fondée sur la théorie et la recherche sur la neuroplasticité et la sensibilisation centrale, la PRT propose une relecture de la perception douloureuse comme un signal potentiellement sans danger. Des essais randomisés contrôlés récents, dont un avec un suivi à long terme, montrent une réduction importante et durable de la douleur. Cet article résume la recherche actuelle, les bases conceptuelles, les formations existantes, les techniques cliniques et l’intégration de la PRT dans la pratique, avec un exemple tiré de la pratique au CHUV.
